Application process
If you are aware of an individual (child or adult) with disabilities who is being abused or neglected, please report this information as soon as possible to Unusual Incident Coordinator, Kelly Jackson at 740-453-4829 during business hours (Mon. - Fri. 8:30 am - 4 pm). On evenings, weekends, or holidays call our emergency number at 888-905-0787. Reports may be made anonymously if you choose.

Providing information in order to make you aware of how to get help and what to report when a person with developmental disabilities is thought to be involved in incidents that adversely affect their health and safety. These incidents may include but are not be limited to the following:
  • Physical-physical pain or injury
  • Verbal-language or gestures that threaten, insult, or show disrespect
  • Mental-harassing or humiliating
  • Sexual-inappropriate touching or sexual gratification
  • Neglect-failure to provide food, medical, or personal care
  • Violation of Rights-basic client rights are ignored
  • Misappropriation (theft)-obtaining the property of an individual without consent.
