Provides transitional care services to youth. Services include community referrals, assistance in locating housing, training in daily living skills and assistance with benefits. Teams help emancipate clients from the children’s system into mainstream independent living. Those with severe psychiatric disorders are helped to make a transition to the adult system of care.
