Cuyahoga County Options For Independent Living | Cuyahoga County Division Of Senior And Adult Services

Provides case management and coordination of needed home care services in the following areas: medical transportation, emergency response system, personal care, homemaker services, home delivered meals, and chore services, including laundry service. Services can be requested individually or combined. Can provide limited home bathroom modifications for safety. Does not include skilled nursing services. When program is full, assists those on waiting list to find needed services in the community. Chore services and limited bathroom modifications cannot be assessed without using other services.

Application process

Phone for application information and telephone screening. Following this, there will be an in-home assessment for applicant. Client can apply for a single service if desired, but they will be advised if they may also be eligible for other assistance if they choose. NOTE: Extremely long waiting list, basically, as someone comes off the program the agency will then let an applicant enter program (1 out, 1 in.) There may be periods when the agency is not accepting applications for particular services through the program.

Fee

There is a co-pay for this program; clients pay a percentage of the services they receive. That percentage is based on the participant's income.

Eligibility

Open to frail elderly age 60 or older, or those 18 through 59 with a documented disability who are living in a private home or apartment in Cuyahoga County (excludes group homes, assisted living residences, and nursing homes) and are not eligible for any Medicaid waiver program. Eligibility is also based on an assessment of need for activities of daily living assistance such as bathing, dressing, meal preparation, housekeeping etc.. Participants must be below 300% of the federal poverty level.
