Eligibility

Targets people of low-income (below 150% of the Federal poverty Guidelines) with physical or emotional disabilities aged 18 and over or ages 55 and over who are receiving or are eligible for Medicare. No residency restriction, but focuses on those living in Cuyahoga County.

Required documents

Documents are not required, but the following information is needed: Medicare ID number, Social Security Number, list of medications, pharmacy name and location, annual or monthly income, dollar value of assets (including stocks, bonds, cash value of life insurance policy, savings, etc.) and the same information for a spouse, if applicable.

Description

Seeks to ensure that Medicare beneficiaries with limited incomes and resources know about and make the best use of all available resources for reducing the costs of prescription drugs. Offers an information line to help people understand, compare and sign-up for the Medicare Part D Prescription Drug Benefit and the Low Income Subsidy. Works with beneficiaries to resolve Medicare Part D problems. Also helps those who are already receiving Medicare Part D benefits and want to select a different plan for the next calendar year. Also offers benefits screening for other government programs through Benefits Check-up.
