Students need to bring necessary paperwork provided by the school sports program. Students are also encouraged to bring their immunization records. If students are not accompanied by a parent, they should have their paperwork signed by a parent prior to attending the clinic.

Accepts Medicaid, Medicaid Managed Care Plans, Medicare, insurance plans, or self pay. Patients that do not have insurance will be eligible for sliding fee scale with proof of income. Out-of-pocket expenses can be reduced depending on income and family size. Costs are significantly lower than going to ER or urgent care. Significant cost reduction for any Lab work and prescriptions. Regardless of payer source, NO ONE is turned away.

The Community Health Center is a patient-centered medical home offering preventive and primary care to for men, women and children of all ages, including those who have no insurance, are under-insured or have insurance. Services include medical, dental, addiction treatment, mental health counseling, as well as women's health, well child visits, exams for work, school and sports MEDICAID Patients welcomed.
