Eligibility
Must meet one of the following criteria:
- Older adult age 65 and over.
- Individual with a disability.
- Individual receiving SSDI.
- Individual with a Medicare card.
Required documents
Must provide a photo ID and one of the following:
- Current SSDI benefit letter.
- Documentation of Medicare benefits.
- Documentation of disability.

- Monday - Friday: 8:00am to 4:30pm.
