Eligibility
Serves low-income individuals residing in the United States. Will not help with any past medical bills, including items or services that have already been ordered. Will NOT accept self-referrals or referrals by family/friends. Applications must be submitted from a referring professional.
Required documents
Completed application form. Attach a written vendor estimate and a letter from the applicant's physician describing his/her overall health and need for the requested item.
Hours
Accepts applications for assistance throughout the year.

Application process
  • Application must requested online by and submitted by a referring professional such as school personnel, a counselor, a teacher, a nurse or health aid, clergy/pastor, human resources staff, senior center staff, hospital staff, social service caseworker, social service technicians, or staff and volunteers at human service agencies.

  • Orthopedic braces and shoes.
  • Prostheses.
  • Stairlifts.
  • Vehicle adaptations, hand controls (no van conversions).
