Office hours: Mon-Fri 8:30 am-4:30 pm. Appointments available and necessary during any federally and state mandated regulations. Call for phone evaluation to determine required documentation based upon request. After evaluation, staff member will provide instructions, requirements, and schedule an appointment to facilitate the process.

A non-residential agency offering services for individuals with disabilities that will assist them in remaining active and independent within their homes and communities. Provides the following core services as defined by Administration for Community Living for Individuals with Disabilities: Information and Referral, Peer Support, Advocacy, Independent Living Skills Youth Transition, Institutional Transition, and Diversion Services. Working for, and WITH, individuals with disabilities in the North Central Ohio area included: Richland, Crawford, Morrow, Knox, Ashland and Huron counties.
