Individuals with disabilities requiring assistive technology.

Hours

Office hours: Mon-Fri 8:30 am-4:30 pm. Appointments available and necessary during any federally and state mandated regulations. Call for phone evaluation to determine required documentation based upon request. After evaluation, staff member will provide instructions, requirements, and schedule an appointment to facilitate the process.

Call for information, instructions and availability of services and funding. Consumer Intake packet must be submitted with appropriate documentation.
