Individuals with disabilities requiring assistive technology.
Required documents
Completed agency intake, photo ID, disability verification and program registration form.
Hours
Office hours: Mon-Fri 8:30 am-4:30 pm.
Appointments available and necessary during any federally and state mandated regulations. Call for phone evaluation to determine required documentation based upon request. After evaluation, staff member will provide instructions, requirements, and schedule an appointment to facilitate the process.
