Fee

Accepts various third-party payments (Passport, Options, Board of Developmental Disabilities, Veterans Affairs, Long-term care insurance, etc.) and certain individuals may qualify for a sliding-scale fee structure. If not qualified for sliding-fee or third-party payment, there is a fixed fee for services.

Eligibility

Serves Cuyahoga County. Older adults age 62 yrs or older as well as developmentally disabled individuals age 50 yrs or older. Will accept those with mild to moderate cognitive impairments (including Dementia) and people with mobility problems, provided their physical care does not exceed the level that can be provided by staff.

Provides adult day services, assisted living, long-term nursing home care, senior apartments, skilled nursing and short-term rehabilitation, and a variety of other programming for seniors. A wellness center, on site chapel, pastoral care, intergenerational programming and other amenities are available for residents and others who utilize the services.
