Must be employed or attending an accredited school and income is below 145% of poverty level. Ongoing (maintaining continuously) eligibility thereafter is 300% of the Federal Poverty Guideliens. Application, proof of address, driver's license/photo ID, proof of income, Social Security card, birth certificate, proof of employment, and a selected certified child care provider.
