Must be homeless or facing a court ordered eviction for homeless assistance . Income eligibility of 200% of Federal Poverty Level for prescription assistance.

Required documents

Proof of income for the last 90 days, utility bill, ID verification, birth dates and social security numbers. Proof of citizenship
