Fees
All fees are based on ability to pay. No child will be turned away based on guardian's inability to pay. No money is collected at the site, fees will be billed. Please bring child's shot record and insurance card to every visit.

Collaborates with Public Health-Dayton & Montgomery County and offers the following immunizations at this site: DTaP, Polio, Hib, Comvax, Hepatitis B, MMR, Chicken Pox, Tetanus-Diphtheria, Influenza, and Prevnar (on a limited basis for those who qualify).
