Children ages birth through 21 years old. General dental exams begin at 2 years of age, while x-rays, cleanings, or fillings begin at 5 years of age.
First time patients must register and bring: Proof of income (W2 form/tax return OR 2 current pay stubs) Health insurance card ( if applicable) Social security card Driver's license or Photo I.D. and Proof of Dependents (tax document or Social Security Card for any children). Physicals: form from employer, school or agency to be filled out by physician.
M-F: 9:00am-5:00pm extended hours Monday and Thursday generally: 9:00am-8:00pm by appointment only.

Description

Provides a full range of health care services, including behavioral health for infants and children. Services include Immunizations, well visits, school physicals, and same day sick visits. May be able to provide clients with a limited supply of some prescriptions written by Free Clinic physicians. Will also help clients access and complete paperwork for patient assistance programs that provide prescriptions.
