Eligibility
Serves males and females ages 18 and older who meet medical, mental health, and addiction criteria.
Required documents
Medicaid information if applicable. Proof of residency, such as driver's license/ state ID, current utility bill, lease agreement, piece of mail with current address, current election card, automobile registration, pay stub or recent tax return. Additional Information will be given during individual's phone contact with agency.
