Application process
Application required. Applications may be completed on website. As part of the application process, applicants must submit Medical and Eye examination forms completed by their medical providers. Applicants must also provide a video submission demonstrating navigation skills, pace, physical stamina, and areas of use.

Eligibility
Applicants must be legally blind, at least 16 years old, physically and mentally capable of receiving benefit from a pilot/guide dog, and have previously completed Orientation and Mobility training.
