Death certificates are filed in the county in which the event actually occurred. If a death occurs in Perry County a certified death certificate can obtained from the Perry County Health Department. If the death occurred outside of Perry County, but within Ohio, the certificate can be obtained from the County in which the event occurred or from the State Division of Vital Statistics.
