Eligibility
Routinely recommended vaccines are provided to Montgomery County residents from birth -18 years of age.
Required documents
Must bring child's shot record and insurance card to every visit.
Hours
Immunizations are available on a walk-in basis Mon - Fri. 8:00am - 10:30am, 12:30pm - 3:30pm.

Fees
For uninsured individuals, there is a office visit fee and a per shot administration fee. These fees are based on ability to pay. No eligible child will be turned away because they cannot afford to pay the fee. Accept cash or check. For clients with Medicaid and Medicaid HMOs these fees are covered by insurance.
