Appointments preferred, walk-ins welcome. Satellite clinics are walk-in. To schedule an appointment for vaccination, call 419-774-4700.

Fees

Fee per vaccine for other vaccines, $28 for first vaccine and $15 each additional vaccine due at time of service, Medicaid, private pay, and most insurances accepted.

Provides a program for adults needing routine vaccine: Tetanus, Pneumonia, Meningitis, Hepatitis A and B, Chickenpox, Shingles, Human Papillomavirus (HPV), Polio, Measles, Mumps, and Rubella. Immunizations at the Health Department are by appointment or walk-in. Satellite clinics are a walk-in.
