Eligibility
Financial Eligibility:
- Individuals or families with incomes of up to five (5) times the federal poverty level may qualify.
- If income is over criteria, call and discuss individual's financial situation with one of Cancercare's Co-Payment Specialists.
- Must be diagnosed with one of the cancer types that the Foundation covers and the diagnosis must be verified by a doctor. Applicant's doctor must complete and sign our physician verification form.
- Must be a United States Citizen or legal resident who is receiving treatment dispensed in the United States.
- Must be in active chemotherapy or target treatment.
- The prescribed treatments must be approved by the Food and Drug Administration (FDA) for cancer.
- Must be covered by private insurance, an employer-sponsored health plan, Medicaid, TRICARE, or have Medicare Part B, Medicare Part C, or Medicare Part D.
- Must have insurance that covers a portion of medications. Some funds are restricted to assist only those covered by a federal health insurance program such as Medicare, Medicaid, or TRICARE, while others accept both private and federal insurance.
- If the applicant is uninsured (do not have any insurance or medical plan that covers prescription medication), then the individual is not eligible for co-payment assistance; however, the individual can still call the toll-free number for referrals to other organizations or patient assistance programs.
