DIAGNOSTIC PROGRAM | OHIO DEPARTMENT OF HEALTH
Provides funds for an evaluation of a potentially handicapping condition, establishment of a diagnosis, and development of a plan of treatment. May include lab tests, x-rays, specialized diagnostic tests, biopsies, evaluations for physical, occupational and/or speech therapy, community nutrition services, psychological evaluations under some circumstances for children with potential handicapping physical problems, dental consults, consults and office visits to physicians, and inpatient hospitalization up to 5 days. Evaluation must take place within a 6 month period. Families must apply their health insurance, if any, to the cost of services.

Application process
A medical application must be submitted by a BCMH-approved physician, who obtains this form from the agency. Parent or guardian should contact their child's primary care physician to see if they are approved. May also contact a public health nurse at the local health department, the agency's office, or see web site to locate approved providers or for detailed information. Parent, managing physician, and local health department will receive a letter of approval or denial, with explanation.

Free (Insurance or Medicaid coverage must be applied if available; client may in some cases be eligible for some insurance premium reimbursement).
Eligibility
Open to Ohio residents who are permanent U.S. residents or U.S. born, who meet age guidelines, and who may have a medical handicap. There is no financial eligibility requirement for this program. Must be under the care of a Bureau for Children With Medical Handicaps (BCMH)-approved physician to receive evaluation. Only one assessment will be authorized for a particular condition unless there is a substantial change in that condition or a change is made in physician. Does not cover acute conditions, emotional and behavioral problems, routine well-child care, or pre-school, adoption, or sports evaluations.
