Open access (new client) hours Mon-Fri for those who want a mental health or substance use assessment. Call (419) 584-1000 for most current times and important information to bring to the office.Call or email.

Fee

All forms of Ohio Medicaid accepted as full payment. Medicare and private health insurance billed directly; clients are responsible for amounts not covered. Sliding fee scale (based on household size and income) available for residents of Mercer, Van Wert, and Paulding Counties. Payments can be made online, in person or by mail. All major credit cards are accepted for payment.

Foundations Behavioral Health Services is a community-based behavioral health center serving Mercer County and surrounding areas who offers providing professional counseling and support services to local residents. Services available include mental health and substance use services, youth and adult services, individual and group services, crisis intervention services, diagnostic assessments, outpatient psychiatric services with case management and therapy services, intensive home-based services, intensive outpatient program, prevention programs, and drug-free and MHFA workplace trainings.
