Congregate Housing Services Program | Central Ohio Area Agency On Aging
The concept of CHSP is to provide clustered services to residents in the housing complex so that they can have a more individualized care plan. For example, if a person needs assistance with getting up in the morning, or a medication reminder, they can receive that service, and then be visited again later in the day for additional services if needed. The services are provided by a contract agency. The cluster model of services provides the residents the benefit of consistent staff, and a care plan that is tailored to meet their individual needs.

COAAA plans, funds, and delivers services that help older adults and individuals with disabilities remain safe and independent in their homes. With the assistance of area providers, COAAA arranges and coordinates services to help individuals with activities of daily living, such as homemaking, transportation, home delivered meals, personal care, and more. COAAA offers education and resources to caregivers, professionals, and the public. COAAA also advocates for programs and policies that benefit older adults and individuals with disabilities.
