Will need the following information/documentation: date of birth and Social Security Number for self and all those in the household Veteran status for self and spouse length of time in current residence a list of the prescription medications currently taking including dosages current check stubs (if employed) and income from all other sources for self, spouse and others in the household rent or mortgage payment amount current utility bills current cell phone bills. Will be advised if other documents/information are needed.
