Provides medical care emotional support to terminal patients, both in the home and for short-term inpatient stays for pain and symptom control. Also offers a 10-patient residential facility for terminally ill patients who have no one to care for them or whose caregiver can no longer provide care. Service includes skilled nursing care, pain management and symptom control, and home health aide services.

Application process

Phone M-F: 8:00am-4:30pm to get information or to arrange initial home visit by member of the hospice team. A written or phone referral from a physician is required for all new clients.

Fee

Accepts Medicare, Medicaid, and private insurance. Residential: accepts Medicare, Private Insurance. Fee for the room portion of residential care is not covered through third-party payment and is private pay.

Eligibility

IN-HOME/SHORT TERM CARE: Patient must have a terminal illness with a limited life expectancy, and must no longer be seeking curative, aggressive, invasive therapies. Must have an identifiable caregiver or plans to arrange for one for future needs. Must have a physician who will continue to assume responsibility for medical management. FOR RESIDENTIAL CARE: patients must be 18 or older, and must have a terminal illness with a limited life expectancy, and must no longer be seeking curative, aggressive, invasive therapies. Client must be in a situation where no caregiver is available (e.g., client has no family, caregiver is ill or elderly and unable to provide care, client needs more care their can be provided in the home, etc.). Clients with certain illnesses, such as very active Tuberculosis or very intensive shingles, may not be eligible.
