First time patients must register and bring: Proof of income (W2 form/tax return OR 2 current pay stubs) Health insurance card ( if applicable) Social security card Driver's license or Photo I.D. and Proof of Dependents (tax document or Social Security Card for any children).
Thursdays from 10:00am-12:30pm. By Appointment: M-F: 9:00am-5:00pm and select evenings and Saturdays.

Uptown Office: Walk-in for service. STD testing is also available by appointment Monday through Friday from 9:00am-5:00pm and select evenings and Saturdays, phone to make an appointment. Patients with scheduled appointments will be prioritized for their appointment. East and West Offices: phone for an appointment M-F: 8:30am-4:30pm.

Third-party payment accepted, including Medicaid and Medicare. Financial Counselors on site assess payment based on income for those without insurance. Sliding fee scale rates are applied based on income, scales goes to zero.
