Referral must be from a church, social worker, case manager, hospital, community clinic, or doctor's office on the agency's letterhead. Referral must be used within 24 hours or the next business day. Call if the referral can't be used within that time limit. May only use once every six months.

Social Security cards, birth certificates, school IDs, or medical cards for all other members of the household
