Community Clinic | Licking County Community Health Clinic
Clinic provides primary health care to persons, ages 18-64, who do not have a family physician and/or are low-income without medical insurance. If you have a medical emergency when the clinic is closed, please go to Licking Memorial Hospital. If you have a dental emergency, please call 2-1-1. We do not treat pain, women's health or STDs. There is a dental clinic in Zanesville that we can refer you to.

Please call 345-1113 one hour before clinic times to schedule appointments. All patients should call to schedule an appointment so you can be screened before arriving at our building/office. Fee
$10.00 donation. We do not accept private insurance or Medicare. We DO accept Medicaid, however, we will not bill them.
Eligibility
MUST be a Licking County resident, age 18-64. Must provide photo ID, utility bill, verification of address. Must be at 300% of the poverty level. We utilize HOUSEHOLD income. Individuals can gross $700.00/week; Families of 2 can gross $950.00/week; Families of 3 can gross $1199.00/week; Families of 4 can gross $1448.00/week. Add $249.00 for each additional member of your family.
