Functioning For Life | Cleveland Clinic Foundation

Provides shared medical appointment sessions which are designed to work in conjunction with clients' primary care physicians to reverse disease for individuals who have a diagnosis of a number of chronic, debilitating, medical conditions, including lupus, psoriasis, . The program teaches how to manage and reverse chronic disease by learning stress management techniques, exercising, cooking healthfully and understanding nutrition.

Eligibility

Ages 18 and older, with a diagnosis of prediabetes, type 2 diabetes, high blood pressure, or high cholesterol, autoimmune/disorders such as lupus and psoriasis, women's health disorder, digestive disorders, and pain management who have registered for the program as a Cleveland Clinic Patient. The primary care physician need not be a Cleveland Clinic doctor.
