Patients under the care of a physician who feels that diet counseling or medical nutrition therapy would be helpful.

Application process

Phone M-F: 9:00am-3:00pm for appointment. New patient must have a prescription from a physician with the diagnosis.

Fees

Fixed fee. Generally not covered by insurance, though some diabetic and pre dialysis kidney disease patients may be covered by some insurances and Medicare.
