Eligibility
  • Serves student athletes in Tuscarawas County in middle, high school or college.
  • College students should check with the college to ensure sports physical may be completed by a nurse practitioner.
  • Minors must be accompanied by a parent/guardian.

Application process
Call 330-343-5555, extension 1810, for an appointment Monday and Wednesday from 8:00am to 4:00pm, Tuesday and Thursday from 8:00am to 7:00pm, and Friday from 8:00am to Noon.
Fees
  • Sports physicals will be provided for $45 for persons up to age 26. Payable by cash, check, Master Card, Visa and Discover credit or debit card.
  • During the sports physical days, immunizations will also be offered at an additional charge. Immunization fees will be billed to medical insurance or be charged an administration fee per vaccine for those who are income eligible.
