Transportation To Medical Appointments | Molina Healthcare Of Ohio

Provides 15 round-trip visits (30 one-way trips) for each member per calendar year to any network provider. Medical appointments include trips to a doctor, provider, clinic, hospital, therapy or behavioral health appointment. Unlimited rides for pregnancy and well-child visits. Immediately following a medical appointment, provider will cover trips to the pharmacy to pick up a prescription. These trips are limited to provide transportation to and from any WIC or County Department of Job & Family Services (CDJFS) re-determination appointment, as an additional service.

Application process

Phone to arrange transportation no later than 48 hours (two business days) before an appointment. The hold on these reverifications has ended and reverification reminder notices will be rolled out throughout the year. Enrollees should look out for reverification packets in the mail and report any address changes to their JFS office.
