Eligibility
- Serves older adults age 65 and over and individuals with disabilities in the City of Macedonia.
- Household income must be less than $46,080 for two (2) people or $33,975 for one (1) person.
- Must abide by all other regulations adopted by the city administration.
Required documents
Must provide copy of photo ID and proof of income including previous year's tax return, Social Security statement, pension statement, and current bank statement. Applicants under 65 must provide a physician's note of disability verification.
