- Must be at least 18 years old or older - Willing to collaborate and one of the following: - 2 psychiatric hospitalizations within 12 months with the involvement of another system Examples of another system; criminal justice, homelessness, veteran, Board of developmental disabilities, 65 years or older, other Or… - 2 or more crisis stabilization unit stays within 12 months with the involvement of another system. Examples of another system; criminal justice, homelessness, veteran, Board of developmental disabilities, 65 years or older, other Or… - 1 inpatient psychiatric hospitalization and 1 crisis stabilization unit stay within 12 months with the involvement of another system Examples of another system; criminal justice, homelessness, veteran, Board of developmental disabilities, 65 years or older, other Or… - Currently involved in an outpatient competency restoration program ( does not require hospitalizations or crisis stabilization unit stay or another system) Or…. - Currently incarcerated in jail and diagnosed by a licensed clinician with a significant mental illness and in need of supports upon release( does not require hospitalizations or crisis stabilization unit stay or another system)
