Conducts a comprehensive, diagnostic evaluation and assessment which identifies each client's problem. This assessment is used to recommend a method of treatment to best meet the individual's needs. Includes taking psychosocial information, diagnostic testing and collateral data from family, employer, etc.

Phone for an appointment at any of the sites. Calling hours by site are: Glenbeigh Hospital, 24 hours a day, 7 days a week; all other sites M-F: 8:30am-5:00pm.
