Families with child ages Birth-21 who have multi-system needs, are at risk of out-of-home placement, or returning home from out-of-home placement. Families with private insurance or no insurance can be served. OhioRISE eligible children who are Tier1 or who do not wish to receive the OhioRISE Care coordination services are also eligible.

The Council Office hours are 8am-4:30pm Monday through Friday. Council meetings are held the first Wednesday of August, October, December, February, April, and June, beginning at 1:30pm at the Licking County Health Department. Visit our website, lcfamilies.org, for directions to attend the meeting virtually.

Description

Community Support Teaming is available to families with youth ages 0–21 who have multi system needs, are at risk of out of home placement, or are transitioning back into the community after such a placement. Through the High-Fidelity Wraparound model, Team Facilitators partner with families to build a family team and strong support network, connect them with community resources, and coordinate services among providers involved in the child’s care. This approach is rooted in strengthening families and enhancing the local social and human services system. By sharing resources, improving coordination, and fostering collaboration across agencies, Community Support Teaming works to ensure that children, youth, and their families receive the comprehensive support they need to thrive.
