Eligibility
  • Age 18 or older.
  • Reside in Summit, Cuyahoga, or Medina County.
  • Participating in substance use disorder treatment.
  • Have been diagnosed or participated in SUD treatment in the last 10 years.
Required documents
  • Current driver's license or state ID.
  • Birth certificate.
  • Social Security card.
  • Proof of SUD treatment.
  • High School diploma, GED, or transcripts (if applicable).
  • Proof of residency.
