Referrals to the program accepted from: - ODRC Unit Management Chiefs or other ODRC program staff. - Ohio MHAS Community Linkage Social Workers. - Homeless shelters and homeless outreach workers. - Correctional halfway house staff. - Ohio Department of Health Community Linkage Coordinators. - Other local reentry program providers.

Eligibility
- Serves individuals incarcerated in an Ohio correctional institution (a prison) in prerelease planning who are at risk of homelessness and identified as having a mental illness or HIV diagnosis. - Serves individuals previously incarcerated in an Ohio correctional institution (a prison) less than 120 days after release who are at risk of homelessness and identified as having a mental illness or HIV diagnosis.

Provides community-based behavioral and physical healthcare services in Summit County and the surrounding region. Serves as a safety net for those who suffer from illnesses such as schizophrenia, bi-polar disorder, major depression and personality disorders. Services include psychiatric and medical programs, rehabilitation, advocacy, and support for recovery. Agency is a Certified Community Behavioral Health Clinic (CCBHC).
