Must complete a complaint form (form DTE 1). Forms must be dated, signed, and notarized. Online electronic submissions do not need to be notarized.
Hours
Complaints are accepted from January 1st until March 31st of each year during regular office hours Monday through Friday from 7:30am to 4:00pm. Forms must be received or postmarked by March 31st.
