Utility Disconnection Protection (MEDICAL CERTIFICATE) | Duke Energy
Provides a 30 day extension to prevent utility disconnection with verification of qualifying medical condition from a medical provider. A maximum of three medical extensions can be used within a rolling 12-month period.

Ohio customers may complete a Medical Certificate Request Form online at https://www.duke-energy.com/home/billing/special-assistance/medical-certificate-request Kentucky customers should contact Duke Energy Customer Service by phone to request a Medical Certificate Form be faxed to their medical provider.
