Application process
A medical application must be submitted by an agency-approved physician, who obtains this form from the agency. Parent or guardian should contact their child's primary care physician to see if they are approved. May also contact a public health nurse at the local health department, the organization's office, or see web site to locate approved providers or for detailed information. Parent, managing physician, and local health department will receive a letter of approval or denial, with explanation.

Eligibility
Serves Ohio residents age 21 or older who have been diagnosed with cystic fibrosis. Client must be under the care of a Bureau for Children With Medical Handicaps (BCMH)-approved physician.The family must be financially eligible based on the service needs of the patient and the adjusted gross income of the family. This is decided on a case-by-case basis. Case can be renewed if medical and financial eligibility continue.
