Eligibility
Serves adults ages 18 and older with developmental disabilities in Medina County.
Required documents
Will be requested as needed and include:
  • Funding documentation (waiver or private pay).
  • Medical records.
Hours
Monday - Friday: 9:00am to 3:00pm.

Application process
  • Call the office for more information on private pay options.
  • If using a waiver, must be referred by the Board of Developmental Disabilities or Medicaid's Intermediate Care Facilities for Individuals with Intellectual Disabilities program (ICF/IID).
Fees
  • Accepts Individual Options Waiver, Level One Waiver, and SELF waiver.
  • Fixed fee for private pay.

Provides a social, recreational, and educational day program that offers courses in art, music, computers, cooking, physical fitness, community awareness, vocational skills. NOTE: A portion of course instruction is assisted by licensed therapists.
