- Must be employed at least 20 hours a week. - Serves pregnant women and families with children under the age of 18. - Applicants must be residents of Portage County. - Total household income must be at or below 200% of the Federal Poverty Guidelines. - Client will receive notice of approval or denial within 30 days.
Provide verification of all sources of household income (unless enrolled in SNAP/OWF), birth certificates, and Social Security numbers for all household members. Also, provide verification of qualifying employment. Other documents will be requested as needed.
