OHIO HOME CARE WAIVER | DIRECTION HOME AKRON CANTON AREA AGENCY ON AGING AND DISABILITIES

Provides a Medicaid Waiver program that offers care management to help participants access nursing services, personal care assistance services and/or skilled therapy services, one or more waiver specific service such as: home modifications, home-delivered meals, adult day health care, respite care, supplemental transportation, adaptive/assistive devices, and emergency response systems. It is designed to meet the needs of consumers eligible for Medicaid who have been assessed to require an intermediate or skilled level of care. Without the services available through the Waiver benefit, these consumers are at risk for hospital or nursing home placement.

Serves financially eligible individuals age 59 years and under residing in Summit, Portage, Stark, or Wayne county who have been assessed to require an intermediate or skilled level of care.

DIRECTION HOME AKRON CANTON AREA AGENCY ON AGING AND DISABILITIES

Plans, funds, and coordinates services for older persons and persons with disabilities in Portage, Stark, Summit, and Wayne counties to live with dignity and choices in their homes and communities for as long as possible. Administers PASSPORT, MyCare Ohio, The Assisted Living Waiver, The Ohio Home Care Waiver, Care Coordination, Elder Rights, Family Caregiver Support Program, Nursing Home Placement Assistance Program, and other services designed to help older adults and individuals with functional limitations maintain independence. Provides funds for services administered by other community agencies including client finding, specialized information and referral, transportation, home-delivered meals, medical treatment, personal care, respite care, safety monitoring, health assessments, congregate meals, recreational activities, and adult day care.
