Eligibility
Those 18 and older struggling with any form of substance use addiction.
Required documents
Photo I.D.
Proof of income, and Medicaid information (if applicable). Will be advise of any other documents needed during intake.
Hours
M-F: 8:00am-5:00pm.

Accepts Medicaid and self-pay. May provide a sliding scale for those without insurance.
