BIRTH AND DEATH CERTIFICATES | ZANESVILLE-MUSKINGUM COUNTY HEALTH DEPARTMENT
ZMCHD can now issue birth certificates for individuals born anywhere in Ohio after 1908. This eliminates the need to visit the county of birth to obtain a certificate.

Fee
$22 certificate fee. Pay by credit/debit card by submitting an online request through zmchd.org OR visit our office at 205 N. 7th St in Zanesville OR print our a request form from zmchd.org and mail it with payment to ZMCHD.
