Case/Care Management For Mental Illness/Emotional Disabilities | Northkey Community Care

Provides case management services to assist individuals with recovery related to mental health conditions. Case managers help participants evaluate service needs, access services, monitor services being received, and coordinate existing or planned services.

New patients may schedule an appointment in advance by phone or walk-in for their initial visit. Ongoing services are provided by appointment.

Open to residents of Northern Kentucky age 18 and older and children diagnosed with a severe emotional disturbance (SED).
