Youth ages 21 and younger who have medical hair loss. Must meet income eligibility, inquire for details.
Completed application two letters of recommendation (may be from parent, teacher, coach, troop leader etc.) current photo of the child without his/her hairpiece financial information for individuals responsible for child such as the most recent tax return or proof of public assistance.

Children may be nominated for services by a parent or other adult such as a health care professional, social worker, etc. To nominate a child for services, go to the web site or phone for an application. Complete the application and send it by certified mail along with all required documents. Children who are accepted to receive services will be notified.
