Description
Links individuals to mental health services and provides outreach in the streets, under bridges, in wooded areas, wherever they can find individuals who are experiencing homelessness and living with mental illness. Once contact is made, assistance such as food, blankets, clothing, hygiene supplies, crisis intervention, peer support, and transportation to intake appointments is offered.
