Eligibility
HIV positive status. Household income must not exceed 300% of Federal Poverty Level. Reside in our service area. Documentation of proof of crisis. The HOPWA program is not available to people living in subsidized housing or people who have accessed the PIPP program. Strict eligibility guidelines: -REQUIRED: documentation of proof of attempted assistance at another local agency for rent/mortage &/or utilities PRIOR to applying through Equitas Health (EX: Catholic Charities, Community Action, Salvation Army) -CANNOT be receiving any form of subsidized housing -CANNOT be receiving/or have accessed PIPP assistance
Required documents
Documentable proof of crisis (need). Documents for determining eligibility: Identification, medical status (HIV positive), proof of county of residence (utility bill, award letter, lease agreement) and income verification (pay stubs, award letter, etc). Other documents required to complete assistance request will be determined at appointment.

HOPWA assists Equitas Health clients and any HIV positive clients in paying rent, mortgage, electric, gas and water bills to prevent homelessness or interruption of utility service in an emergency situation. Case management services can include: budgeting, counseling, education, referrals to resources, advocacy, and transportation.
