Patients are referred by physicians or discharge planners (social workers, case managers) at the acute care hospital or other health care facility where they are being treated.

Provides intensive rehabilitative services for patients who are disabled as a result of injury or disease. Services include evaluation of abilities and implementation of a rehabilitation plan.
